WSLHD Online Survey Participant Information and Consent

The 2kg Challenge: Evaluating a Public Health Initiative to Address Pre-Diabetes in Western Sydney
Thank you for your interest in this research project run by Western Sydney Diabetes, part of Western Sydney Local Health District (WSLHD).

Why is this research being done?

This study aims to understand:

  • How the 2kg Challenge helps people change health habits
  • What parts of the program work best for participants
  • How the program can be improved and shared with other communities

What will I have to do if I take part?

If you agree to take part, you will complete two short online surveys:

  1. Before the program – about your health and habits
  2. After the 8-week program – to see what may have changed

You may also be invited to complete optional follow-up surveys after the program.

 

About the first survey

The survey:

  • Takes about 10-15 minutes
  • Includes questions about age, health, habits and confidence in preventing diabetes

You will also be asked to provide:

  • Weight
  • Height
  • Waist measurement (halfway between the lowest rib and the top of the hip)

Please have a scale and tape measure ready if possible.

After 8 weeks, you will be invited to complete a follow-up survey. Participants who complete it can enter a draw to win a cookbook!

 

Do I have to take part?
  • No. Taking part is voluntary. You can choose not to participate or stop at anytime without penalty.
  • If you withdraw, you can request that your information be removed from the research data where possible. Once results are combined and analysed in group form, removal may not be possible.
  • Completing the survey is required if you want to join the 8-week program. If you choose not to complete the survey, you will not start the program but can still explore other resources on the 2kg Challenge website here.

 

How will my information be used?
  • Some personal information (like your email) is collected to send surveys and link your responses over time.
  • Your information will not be shared publicly or published in a way that identifies you.
  • Your survey answers will be stored as coded data, meaning your contract details are stored separately from your responses.
  • Results will be analysed and reported in group form, so individuals cannot be identified.
  • Your information will be stored securely according to NSW Health policy for 5 years, and then deleted.
  • Your survey data may be linked with health or program data to see how well the program works.
  • Your information may also be used in future approved research, reports, presentation, or publications.

 

Will I be contacted after the program?

We may contact you over the next 24 months (for example at 3, 6, 9, 12 or 24 months) to ask about your health.
Participation in these follow-ups is optional.
